NOTIFICATION AND CONSENT STATEMENT (“STATEMENT”) In this Statement, a reference to “Organisation”, “Family Centered Support Services” , “Family Centered Ltd” ,“we”, “our” or “us” includes Organisation Group and its current and future related bodies corporate and affiliates. “You” or “your” refer to the person or persons giving this consent. Described below are the terms and conditions for obtaining access to certain parts of or making use of certain features of the Family Centered Support Services Portal.

Please read this information carefully and thoroughly.

Family Centered Support Services is committed to ensuring that personal information collected, regarding candidates, recruits, employees, volunteers, students, and agency employees, is dealt with in accordance with the Information Privacy Principles contained in the Information Privacy Act 2009 and the Australian Privacy Principles contained in the Privacy Act 1988 (Cth) and to protecting the privacy of personal information that the organisation collects, holds, and administers. Personal information is information, or an opinion, about an individual, which directly or indirectly identifies them.

Policy

Family Centered Support Services collects and administers a range of personal information for the purposes of recruitment, employment and ongoing support of employees. The organisation is committed to protecting the privacy of personal information it collects, holds and administers. Family Centered Support Services recognises the essential right of individuals to have their information administered in ways which they would reasonably expect – protected on one hand and made accessible to them on the other. These privacy values are reflected in and supported by our core values and philosophies. Family Centered Support Services is bound by laws which impose specific obligations regarding the handling of information. The organisation has adopted principles as minimum standards in relation to handling personal information. Family Centered Support Services will take disciplinary action for any breach of privacy found, this may be in the form of a Performance Improvement Plan/Performance Improvement Notice or dismissal.

 

 

What will your information be used for?
The information you provide on the Job Portal is collected by us for the purpose of considering you for employment or to update you on employment opportunities.
If your application is successful, we will retain your information as part of your employment records. If your application is unsuccessful, we may retain your information on file and may contact you if a suitable employment opportunity arises or if there are other positions available that may be of interest to you.

What information do we need from you?
To be considered for employment or to be updated on employment opportunities, you will be asked to provide us with personal information. This information includes but is not limited to:

  • Your name
  • ​Address
  • Contact Details
  • Resume
  • Education background
  • Employment History
  • Referees
  • Results of Police Checks

If you choose not to provide any of the information requested, we will be unable to send you information, fully process your application or properly consider you for employment.
All information submitted to us must be true and correct. You must not post or upload inaccurate, incomplete, false or misleading information.

Family Centered collects information required for its primary function of providing quality residential care using a suitable direct care team

Personal information collected is stored on a secured, passworded system and is used initially, to gather information during, and make suitability decisions on and through, the recruitment process and then, for ongoing employment purposes and supervision.

Family Centered does not sell your information or move it or use it outside of Australia.

How your information will be collected?
We collect information from you when you complete the online application form as well as at other stages during the selection process. Any competency testing, medical form and your references are also collected. We require this information to be able to consider your application within our organisation and we will be unable to contact you to advise of your progress with the selection process without this information.

Storage and access of your information:
Your on-line information will be held by a Third Party Data Centre, and is only available for access by us. No access will be provided to any other outside party. Should you wish to change any of your details, this must be done online through your candidate profile at https://familycentered.elmotalent.com.au/careers/portal/.

It is your responsibility to keep your information current.

Family Centered Support Services collects, uses, and stores information of its employees, volunteers, students, and agency employees, received from such, and other bodies, during the recruitment process and ongoing employment, in line with the Information Privacy Act 2009 and Privacy Act 1988 (Cth), including the following:

  • Personal identifying, information, location information in work vehicles, and contact details;
  • Information obtained during recruitment including Curriculum Vitae, reference checks; Citizenship/right to work information, Driver Licence, Licensing and Blue Card clearances, and health information;
  • Training, Education, and Qualifications;
  • Banking, Tax File Number, and Superannuation information;
  • Screening, criminal history, and Blue Card information;
  • Supervision and development documentation.

 

Who your information may be disclosed to?
We may disclose the information you submit to referees, related companies and entities, recruitment agencies, and other relevant organisations who perform services on our behalf during the recruitment process.

The disclosure of your information is for the purposes of processing your application for employment and/or identifying other employment opportunities for you.
Other than as stated here, we will not disclose personal information you provide to any outside party, except where you provide your express permission to do so.
Any offer of employment will be subject to you obtaining all necessary clearances, licenses, training and permits required for your position.

Notifiable Data Breaches

Family Centered ensures that any breaches, or loss, of personal information, which result, or are likely to result, in serious harm, are reported to the Office of the Australian Information Commissioner, in line with the Privacy Act 1988 (Cth) and the Office of the Information Commission as per the Information Privacy Act 2009.
